《三國志V》是光榮《三國志》系列早期最具代表性的作品之一，很多玩家也把它視為 DOS 時代《三國志》玩法風格的集大成之作。遊戲同樣以東漢末年至三國時代的群雄爭霸為舞台，玩家將以君主身分經營都市、登用武將、推動內政、處理外交、發動戰爭，並在群雄割據的局勢中一步步擴張版圖，朝全國統一邁進。本作最突出的特色，是把「名聲」設計成影響整體節奏的重要機制：君主是否獲得民心，會直接影響每月可執行的行動數，因此不只是打仗強不強，連施政是否得民心都會左右整體戰略。戰鬥方面則加入了十二種陣形，以及更多足以左右戰局的武將特殊能力，使得用兵思路更有深度；再加上新增「黃巾之亂」劇本，整個時代範圍也更加完整。後來的 Windows、Saturn 與 PlayStation 版本還採用了電視劇《三國演義》的影像作為開場與過場演出，讓本作在歷史氛圍上更具戲劇張力，也因此成為許多老玩家記憶中特別鮮明的一代。
