1914年7月一戰前夕，東方列車踏上了從巴黎開往君士坦丁堡的最後一趟旅程，玩家化身為美國醫生羅伯特·凱斯，應摯友泰勒·惠特尼之邀，登上東方列車與其會合。 為了弄清楚為什麼泰勒嚴詞要求自己登上東方列車，羅伯特·凱斯發現自己的摯友死在車廂裏，他身上攜帶著一幅神秘的俄文卷軸。 從隔壁車廂到餐廳，對火車上的所有車廂一一進行調查。
